What do I need to bring with me to my exam?

Please bring the following: current vision and medical insurance cards / info, eyeglasses and contact lenses, and current list of medications.

What insurances do you take?

VSP, Eyemed, Medical Eye Services, Tricare, Medicare, Aetna, Blue Cross, Blue Shield, Blue View, Healthnet, Motion Picture, United Healthcare, and Cigna. We do not participate in any HMO plans. If you have questions regarding your insurance it is important to have the primary policy holder’s name, date of birth, and last four numbers of their social security number so we may easily find your policy and detailed information on your specific coverage.

What is a contact lens fitting fee or evaluation charge?

Contact lenses require extra time for the staff and the doctor. Most insurances require an additional charge for the evaluation of contact lenses. The amount of the contact lens evaluation fee depends on the type of lenses you wear and the time associated with fitting or evaluating them. Contact lenses must be evaluated yearly and therefore there is an additional fee which will be assessed during the yearly comprehensive eye exam for patients who wear contact lenses.
